[CF-metadata] standard way to store lookuptables

Hi Andrea:

To figure out if you are using the right form, its best to send CDL to
this group.
To debug your use of the netcdf-java library, its best to send code,
error messages and a sample file to netcdf-java at unidata.ucar.edu.
