[CF-metadata] contributors to standard name table

Dear all

Many people have helped with the development of the standard name table, by
making proposals or participating in the discussion of them. In recognition
of this we have added a page to the CF website acknowledging all those who have
contributed (so far - I am sure it will grow). Thanks. (Please let me know if
anyone has been missed out.)
