[CF-metadata] Standard name table update

Dear All,

I am planning to update the standard name table on Monday 11th October
to make a number of changes that have been agreed in recent weeks.
Please see below for a full list of the planned changes. If anyone spots

c4_plant_functional_types into the area_type table. A number of other
additions to the area_type table are still under discussion in the
'CMIP5 carbon cycle area_types' mailing list thread.
