[CF-metadata] medium 'atmosphere'

Dear Jonathan,

I agree that we suggest to change the postfix _in_atmosphere to the prefix
atmosphere_, as this is already implemented for names apart from the
moles_of_X.

Accordingly the guidelines should be changed:
Could we distinguish between "local medium" and "large-scale medium"? All
existing in_something phrases would be local and there position would be
postfix. The prefixes atmophere_, ocean_, troposphere_ would be
large-scale media.

For dissolved species I suggest the postfix "local medium"
in_cloud_water or in_cloud_ice. (You are right, this refers to the
concentration in the availabe water, not to the cloud's grid box
fraction.)
